cc-switch provider management: cannot delete, or adding fails

Symptom

This cluster holds several UI problems around providers, request addresses, and session management. The core one: cc-switch protects the last remaining provider and refuses to delete it, because the matching app cannot work without one. If you no longer use a provider such as gemini, turn the related feature off in settings rather than deleting it. The other variants — adding a provider failing, // not being enterable in the request address, a deleted endpoint returning after a refresh, and adding or deleting request addresses not saving — are mostly old-version bugs, some of which maintainers said would be or have been fixed in a newer release.
If the provider you want to delete is the only one for that app, cc-switch will not allow it. Turn the app or provider feature off in settings instead — for example, disable the gemini features if you do not use gemini.
If adding, deleting, entering a request address, or saving behaves oddly, upgrade cc-switch to the latest version. Maintainers said several similar problems would be fixed in a new release.
If the old config survives an uninstall and reinstall and deleting files inside the .config folder does not help, the config is probably held by app state or a higher-priority config. Upgrade and configure again.

- No. It showed up on v3.9.1 on macOS, switching input method did not help, and there were no error logs — it is an input handling bug.
- A deleted endpoint comes back after a refresh. What now?
- That is an old-version endpoint deletion bug. Upgrade, or wait for the maintainers to release a fix.
